It's actually just a pared down version of the exact same intake form that all of our microdosing clients get, which we use as a baseline to start developing some better routines in order to lay a foundation to get the most out of microdosing.

Most of the time, if our clients did nothing else but implement the changes we come up with on the other side of filling out this form, they'd get 80% of the benefits of the entire program just by cleaning up their routines.
